Case Manager


Major Responsibilities:

Provides intake assessment and develops an individualized service plan to address risk factors identified during eligibility process; refers and links clients with program services and community resources; engages youth in interest-based opportunity that occupy them during non-school based hours, exposing them to pro-social peers and positive adult role models; maintains frequent contact with youth and families through home visits, field visits, office appointments and telephone conversations to follow through with services as required by GRYD Prevention Model.

Develops effective relationships to promote service coordination for a caseload of up to 25 clients; monitors client participation in program services; provides intervention to clients and families with crises and conflict that need immediate resolution; facilitates/co-facilitates group meetings for clients in various settings; and prepares/maintains needed documentation of client records and reports and actively participated in outreach process.


B.A. in related field required with minimum 2 years of case management experience working with at-risk youth and their families; working knowledge of family dynamics and communities affected by gangs; effective written/oral communication and engagements skills; crisis intervention skills and work experience with high levels of risks for both youth and their families; flexibility to work evenings and weekends as needed; bilingual/Spanish required.
